TERRE HAUTE, Ind. (AP) - Indiana is set to become the second state in the nation to create a hospital-specific reporting system designed to prevent medical errors. Minnesota was the first state to set up such a system. Early next month, a Web site run by the Indiana State Department of Health will debut that discloses information about medical errors -- including the name of the hospital where the error occurred, the type of error and roughly when the error happened. Governor Mitch Daniels signed an executive order at his inauguration in 2005 mandating public reporting of medical errors. Some of the most frequently reported concerns in other states included severe bed sores and surgery-related errors, such as surgery performed on the wrong body part.
